Burnout in nurses and healthcare workers: why it is so common and what helps
Healthcare consistently shows some of the highest burnout rates of any sector. This guide looks at why the work itself creates that risk, how burnout shows up on wards and in clinics, and what protects staff at individual and team level.
The Copenhagen Burnout Inventory was developed and validated in a study of Danish human service workers, and healthcare staff were at the centre of it. That is not a coincidence. Nurses, doctors, care workers, paramedics, and allied health professionals consistently report some of the highest burnout scores of any occupational group, and the pattern holds across countries and health systems.
This article is about why, and about what the evidence says helps. If you work in healthcare and want a number to start from, the burnout test will give you separate scores for personal, work, and client-related strain in about five minutes.
Why healthcare produces burnout
Every major model of burnout points at the same combination: high demands and low resources. Healthcare work is unusual in how many demands it stacks at once.
Emotional labour that does not switch off. Caring for people who are frightened, in pain, or dying requires sustained empathy. That empathy is a resource that depletes, and healthcare offers almost no natural pauses in which to replenish it. This is the demand the CBI's client scale was designed to capture, and healthcare staff tend to score highest on it.
Workload without slack. Understaffing means that a normal day runs at full capacity and any disruption, a sick colleague, an unexpected admission, a difficult case, tips it into overload. There is rarely recovery time between peaks because there are rarely troughs.
Low control over high-stakes outcomes. Staff are accountable for patient safety while having little say over staffing levels, scheduling, admissions, or the systems they work within. Being responsible for what you cannot control is one of the most reliable predictors of burnout in the research.
Shift work and disrupted sleep. Rotating and night shifts disrupt circadian rhythms, and disrupted sleep undermines the recovery that would otherwise offset the day's demands. Sleep is the single largest lever on exhaustion, and healthcare removes it structurally.
Moral injury. A distinct and increasingly recognised strain: being unable to provide the care you know a patient needs because of time, resources, or policy. Repeated experiences of this erode the sense of meaning that would otherwise sustain people through hard work.
Exposure to trauma and death. Beyond burnout, healthcare staff face secondary traumatic stress from witnessing suffering, and grief that is rarely given space.
How it shows up
Burnout in healthcare has the same core signs as anywhere else: exhaustion that rest does not fix, growing cynicism, and a sense of reduced effectiveness. But it has some characteristic presentations.
Depersonalisation of patients. Referring to people by bed number or condition, feeling irritation when a patient needs something reasonable, dreading particular patients. This is the mind protecting itself from emotional demand it can no longer meet, and it is one of the earliest and most reliable signs.
Dread before shifts. A physical heaviness or anxiety in the hours before work that used to be absent.
Errors and near-misses. Cognitive fatigue increases medication errors, missed observations, and documentation mistakes. Staff who are burning out often notice this in themselves before anyone else does, and it compounds the sense of inefficacy.
Presenteeism. Working while unwell because the alternative is letting colleagues down. This is endemic in healthcare and it prevents the recovery that would stop the decline.
Loss of the reason you came into it. Most people enter healthcare because they want to help. When that motivation goes flat, it is a serious sign.
What protects individuals
The individual-level evidence in healthcare is consistent with the general burnout research, with some specifics.
Recovery between shifts is the priority. Protect sleep as fiercely as the rota allows. Keep a consistent sleep window on days off. Use blackout and white noise if you work nights. Avoid the pattern of using days off to catch up on everything else, which leaves no recovery at all.
Buffers within the shift. Even brief pauses between patients, used deliberately, reduce end-of-shift exhaustion. A breath, a few seconds at the sink, a one-line mental closing of the last interaction. Take breaks. Eat.
A decompression ritual. Something that marks the boundary between work and home: the walk from the car park, a change of clothes, a specific podcast. Consistency matters more than content.
Debriefing. Talking through difficult cases with a colleague or in formal clinical supervision. Regularly, not only after critical incidents. Where formal structures do not exist, an informal agreement with a trusted peer does much of the same work.
Keeping one thing outside work. A hobby, a sport, a friendship group that has nothing to do with healthcare. Mastery experiences unrelated to work are among the strongest recovery experiences, and healthcare workers frequently let them go first.

What protects teams
Individual strategies are important and insufficient. Burnout in healthcare is overwhelmingly driven by organisational factors, and the interventions with the strongest evidence operate at team and organisational level.
Adequate staffing. The single largest factor, and the one least in the hands of frontline staff. Where staffing cannot be fixed quickly, honest acknowledgement of the gap matters more than pretending the workload is normal.
Schedule control. Even modest input into shift patterns, such as self-rostering or protected recovery days after nights, measurably reduces burnout.
Rotation of high-intensity work. No one should carry the heaviest patients, the most demanding wards, or the most exposure to death indefinitely.
Structured peer support and supervision. Teams with regular debriefing, Schwartz rounds, or clinical supervision report lower burnout and lower turnover.
Leadership that names the problem. Managers who acknowledge that the work is hard, who protect breaks, who push back on unsafe staffing, and who model taking their own leave, have teams with lower burnout. Managers who treat exhaustion as a personal weakness have the opposite effect.
Reducing administrative load. Documentation, duplicate systems, and low-value tasks consume time that should go to patients or recovery. Trimming them is one of the most appreciated changes a department can make.
Reading your CBI scores as a healthcare worker
Healthcare workers tend to score high on both the work and client scales of the CBI. If your client score is clearly highest, the strain is coming primarily from the emotional demand of patient contact, and the buffers, debriefing, and rotation above are the priority. If work is highest, the load and structure need attention. If both are high, as is common, you need both, and this is the pattern most worth raising with a manager or occupational health early.
